Are you eligible?

A participant may be eligible to access Care Squared supports if they meet the follow eligibility criteria:

• Participant is aged between 0 – 65 years +

• If the participant is aged between 7 – 16 years a thorough triage process is applied to ensure Care Squared has the clinical expertise within the required location to provide support to meet the needs of the participant.

• Participant has a current NDIS plan, with sufficient funding to support agreed supports provision.

• Participant is located within area of Care Squared support provision.

• Care Squared hold relevant registration within the participant’s location for the required supports.

• Care Squared Supports are provided based on the least intrusive options, in accordance with contemporary evidence.
